Dear PlayJam customer,
Thank you for completing the survey on the PlayJam website last week.

Only customers with the active monthly PlayJam subscription on Dish Network will be able to receive their gift vouchers.

To claim your $10 Amazon.com voucher, send a reply to [email protected] and state the following information:

1. Your Dish Network account number (found on your monthly Dish Network bill)

2. Your name and surname

3. Your address

Only Dish Network customers with active PlayJam subscription will receive their gift vouchers. The account numbers and personal information have to be valid and will be verified by Dish Network before the vouchers will be provided.
Only one gift voucher will be provided per active Dish Network account number with PlayJam subscription.

I got the same email, why couldn't they have put that on the FIRST page of the survey, UGH!

Actually When I Went On It, they requried me to be a subscriber to the playjam channel to actually take the survey
