This appears to be a scam. Keep getting a failure message apologizing about their tech people doing some work. Probably just an evil scammer collecting free info from people.
 
Even free, I wouldn't have food from chipotle after all those folks got sick not too long ago.
 
I received a link. Took me to a form, but there are no labels on the form. What do I type in the 3 spaces?
